Advisory notice pertaining to the protection of personal data.

Advisory notice pursuant to Art. 13 of L.D. June 30, 2003 no. 196 (Code for the protection of personal data) and subsequent amendments and additions.

Pursuant to Article 13 of the above mentioned Legislative Decree laying down provisions for the protection of personal data, we wish to inform you that - with your authorization to the processing - personal data voluntarily provided by you for the activation of the requested services, will be treated by Ser-Vis srl - registered office in Zona Ind. S. Adriano 15/16,50034 Marradi (FI) - which manages the http://www.drgiorgini.nl/ website

Ser-Vis srl undertakes to take appropriate measures to ensure the security and confidentiality of your data, in accordance with the above regulations.

1. Information collection

1.1. This advisory notice pertains to the data collected through the forms available next to the relevant services:

a.  Feedback
b. Tell a friend
c. Promotions
d. User area
e. Newsletter

1.2. Any application for registration to the services referred to in paragraphs e) and f) of section 1 shall be verified by Ser-Vis srl through a system that requires subsequent confirmation by the party who has applied for registration (double opt-in) aimed at verifying that the person who submitted the entry is the true owner of the e-mail address to subscribe to the service.

2. Purposes and methods of data processing.

2.1. The data collected in the cases referred to in paragraphs a), b), c) and d) of section 1.1., shall be processed exclusively for the following purposes:

a) establish relationships with users, in particular in order to respond to requests for information and assistance via the "feedback" form;                                                    

b) send out notices of new initiatives;    

c) perform statistical analysis;                                                                            

d) comply with legal obligations.
 
2.2. The data collected in the cases referred to in paragraphs e) and f) of section 1.1., shall be processed exclusively for the following purposes:

a) provide services
b) send out newsletters;
c) periodically send emails containing promotional material and initiatives, internal announcements and commercial messages; these may also be spread through the newsletter;
d) perform statistical analysis;
e) comply with legal obligations.
 
2.3. The processing will be carried out by Ser-Vis srl either manually or with the aid of electronic or automated instruments, and include, within the limits and conditions laid down by Article 11 of Legislative Decree no. 196/03, any operation or set of operations provided by the same decree, under the term "treatment".

3. Nature of data provision

3.1. The provision of personal data is optional. However, failure to enter even a part of the data required in the fields of the different forms marked with an asterisk will make it impossible for Ser-Vis srl to complete delivery of services.

4. Data communication and dissemination

4.1. The collected personal data will not be released to third parties, except in cases provided by the advisory notice and/or the law, however, always in the manner permitted by the latter.

5. Right of the party concerned

5.1. Personal data correction/updating. Art. 7 of Legislative Decree 196/2003 gives the party concerned specific rights.

Namely:

5.1.1 The party concerned is entitled to receive confirmation as to the existence or inexistence of his/her personal information, even if it has not yet been recorded, and its communication in intelligible form.

5.1.2 The party concerned has the right to obtain information on:
a. the origin of the personal data;
b. the purposes and ways of processing;
c. the logic applied in the case of processing using electronic instruments;
d. the identifying details of the data controller, persons responsible and representative designated pursuant to Article 5, section 2;
e. the subjects or categories of subjects to whom the personal information may be communicated or who may acquire knowledge of it in their capacity as designated representative in the national territory, persons responsible or appointees.

5.1.3 The party concerned has the right to obtain:
a. the updating and correction or, when there is the interest, the completion of the information;
b. the cancellation, the transformation into anonymous form or the block of the information processed in violation of the law, including that which is not necessary to keep in relation to the purposes for which the information has been collected or subsequently processed;
c. the certification that the operations as per letters a) and b) have been notified, as far as their contents are concerned, to the parties to which the information has been communicated or divulged, except the case in which this requirement is impossible or involves a manifestly disproportionate effort compared with the right that is to be protected.

5.1.4 The party concerned has the right to object, in whole or in part:
a. for legitimate reasons, to the processing of personal data, even if pertinent to the collection purposes;
b. the processing of personal data for purposes of sending advertising materials or direct selling or for carrying out market surveys or commercial communication."
